MOYT Fitness LLC – Online Fitness Coaching Liability Waiver

Please read this liability waiver carefully before signing. By signing this waiver, you acknowledge and agree to the terms and conditions described below.

1. Risks and Acknowledgment: I, the undersigned client, understand that participating in online fitness coaching and instruction provided by MOYT Fitness LLC carries inherent risks, including but not limited to physical injury, emotional distress, and property damage. I acknowledge that it is my responsibility to consult with a medical professional before beginning any fitness program and to inform my fitness coach of any pre-existing medical conditions or injuries.
2. Release of Liability: I hereby release and discharge MOYT Fitness LLC, its owners, instructors, employees, agents, and representatives from any and all claims, actions, demands, liabilities, expenses, and any other legal rights or claims of any nature arising out of, or in connection with, my participation in online fitness coaching and instruction, including but not limited to personal injury, property damage, or wrongful death, regardless of the cause.
3. Assumption of Risk: I acknowledge that I am voluntarily participating in online fitness coaching and instruction and am assuming all risks associated with such participation, including but not limited to the risks mentioned in Section 1.
4. Waiver of Legal Rights: I agree not to sue MOYT Fitness LLC or any of its agents or representatives for any liability, loss, or damage resulting from my participation in online fitness coaching and instruction.
5. Confidentiality: I understand that my personal information, including health and fitness-related data, may be collected and used by MOYT Fitness LLC as outlined in their Privacy Policy. I consent to the collection and use of my information as described in the Privacy Policy.
6. Governing Law: This liability waiver shall be governed by and construed in accordance with the laws of the state of [Your State], and any legal action related to this waiver shall be filed in the state or federal courts of [Your County].
